4 v crank pulley?

So I have added PS to my truck and am working on the same for a friend. I have now come across 3 cast PS crank pulleys with 2 v grooves...
What I'm trying to figure out is... What are all the v's for. Starting from the engine and moving forward.
1 Alt, 2 AC?, 3 PS, 4 ???
This would be for a short water pump small block?
The PS pulley lines up on both trucks so I have no idea what one might be running that would require a 4th V belt.
what am I missing?

Air pump? Some ac compressors run 2 belts.
