I bought this out of a 90-00 burb to put into my 62 pickup.

can anyone help me what it is form and maybe where I can get some replacement foam?

That seat is from a 95-00 W/T or base model.

Yep, Cheyenne seat from '95-00, same as in my '95 Burb.
