Does anyone make a replacement for item #12 in this picture? The 50 year old one on my Jimmy tore when I was removing it.

Not reproduced to my knowledge. Grab a sheet of neoprene and carve your own. I did the same using a scalpel iirc though the shape differed as I was working with a 72 MY example.

We haven't seen a repro copy being made yet for the inner #12 but here is a picture showing the GM p/n 3994606 found listed for the 71 & 72 K/5 Blazer models, this pictured OE 72 K/5 fuel tank filler neck-flange and tube assy is in extra nice shape and is currently listed/available in the below linked parts board f/s thread.
