Pull the door panel, you'll see two square holes in the door. Trucks with arm rests had caged nuts in these holes with 1/4-20 threads. I did this on a 67 recently. I welded flange nuts in the holes and drilled corresponding holes in the door panels. I used repro 67 arm rests, the 66 screw holes may not line up.
