it might cost a little money but for safety and insurance sake I would buy new lines that are prebent. A lot of companies offer reto kits for the disc swap. Reusing the old rear brakes lines are not a good idea.
As far as routing if you want to make your own, I would look at the LMC catalog. I wouldn't buy anything from them but their catalogs are good for refence purposes
