It is probably a shifter issue. They have a tendancy to get "gritty" inside. It is a fairly complicated unit for the simple thing it has to do. Generally the only thing that is required is a simple tear-down and clean-up. Put is back together with a little grease between the plates and it will be fine.
It is not too hard to do either. It can all be done from above, if you take out the floor mat/pad and unbolt the floor hump. It only takes a few bolts to get the entire thing out.

It is rare to see a 30+ year old truck that still has the full-time case unconverted though. The conversion kit should only cost about $100 and it is well worth the effort. You will notice an improvement in power immediately and better fuel mileage too. You do have to get under the truck for this one though.....
