On the driver side I had to replace the complete hinge opening. On the passenger I only replaced the lower half that was rusted out. I always leave as much of the original as I can (cutting back to good metal) so that hopefully the alignment is ok. I haven't hung the doors yet but do not think I will have issues I took measurements from the step at the bottom and side to the original nut locations and am less than 1/16th off my measurements. The nut has about 1/4" play in all directions so I'm ok.

The truck brake has a pull hand release that mounts under the dash.
