Not all those parts will be used. They are in there to meet the requirements of numerous applications. Most likely you'll just need two of the sleeves for each shock. They will be the ones sized to slide over the bolt that secures them, and they will be very close to the same width as the eye of the shock. You push them into the rubber eye on each end of the shock. Then bolt them on.
