The rear suburban/panel hinges actually go thru the exterior sheet metal of the doors. I'd look at something like the side door hinges from a mid-60's chevy van. They are also an exposed hinge, but bolt on to the jamb and the door going thru a notch cut in the outer skin.
Just a thought without pictures to see what you have on the other side.
