What T-case do you have? Is it a full time axle? Do you have a lift? Are you sure the noise is comming from the driveline?

I would suggest you jack it up and see if you can find the noise by spinning the tires. You may have to jack up the rear also if you need to spin it in 4wd.

Possible causes:
Broken tooth on a gear in the t-case or differential
bad bearing somewhere
The popping out of gear tends to support that idea.
