Here is the switch. it is in the new GM box but it appears to have been mounted at one time as seen on the mounting holes. I did bench test it and it works as it should with no problems at all. if you want it how does $40.00 shipped sound. be aware that I am shipping this from Canada and it does cost me a little more to get this to you.
