I just sold 2 of those. They are kinda hard to find and usually not complete like yours. Nice score.
The square piece is probably not cut from the frame. Both of mine had that square piece and I removed the carriers from the truck myself.
I can't remember exactly how the jamb nuts work. I think the FSM doesn't say much either. IIRC, the nuts tighten to keep the bolt from pistoning in the frame hole but they don't actually tighten all the way. The tire jambs up against the underside of the frame and provides tension once it's all tightened up.
