Being from New England also I know how hard it can be to find good truck or parts this vintage around here. There's nothing left in the salvage yards.
Rust wise that frame looks very good.... As for the bends, I think a good frame shop could fix them easily. Maybe replace the front cross member !
I would take it to a frame shop and see what they have to say !
