My 69 3/4 ton with a 350 made in December, 1968, came with what I think is the one you refer to as formed steel (see pictures below). This one attaches to intake and exhaust manifold bolts and bridges the valve cover. As I recall, the 1969 service manual shows two types for small blocks. The formed steel for the 350 and a cast version for the 307. The 1970 service manual shows the cast version for 10-30 series and the formed steel for 40-50 series.
