I've seen this on a lot of LS powered "patina" trucks where the owner put the fuel cell in or under the bed and wanted to keep the "patina" and I've seen it on trucks where the fuel cell is under the bed with a hidden filler and the owner wanted to keep the stock look to the cab.

I shaved mine when I did my under bed tank.

I would just install a locking gas cap or just stuff the filler full of red rags (under the cap) so people would see the rags when the cap was removed... but for me, it would actually never be a problem as I never let anyone drive my truck.
