Look in the Classic Trucks Magazine May 2016 issue on page 50. The photos at the bottom of the page will make the light bulb turn bright in the back of your brain on this.
On page 21 Classic industries has an ad showing a similar setup.
On page 81 Pros pick has the hinges in their add for 76.00
I opened up the magazine this afternoon and while reading the put the chain eliminator hinges on a 47/53 article stumbled on those hinges.
I think you can do your own pretty easily after seeing the photos.
