Do you mean stock truck intake manifold? or exhaust manifolds?

If you mean intake that is fine the truck intakes flow quite well. If you mean exhaust manifolds, I would consider headers instead. They will be worth decent power with a cam and 243 heads.

If you have an older lower performance 5.3 with dished pistons then milling .030 off the 243 heads will put compression back to stock levels. On a newer/HO 5.3 with flat top pistons milling the heads .030 will give a nice compression bump.

Over all it should be a pretty decent performer with any of the above possibilities.
