I removed it from a 69 Blazer, the bolt hole locations match perfectly

Thanks for the pics and info SK. Someone else suggested Smittybuilt or Con-Ferr, I guess there were a lot of manufacturers at the time. It fits the bumper profile and bolts to the frame underneath for solid support.
