The shifter tube needs to slide up and down inside the column so that the detent at the lower end will clear. It is real common for the shifter tubes to get sticky and not pull back so as to get it into park. I would try spraying the lower mechanism with a light oil or penetrant and then lubing with a light grease. At the shift lever spray a penentrant into the hole where the shifter lever goes into the bezel and work the lever as you spray. Usually you can hit the slide area that needs lubing.

If the shift tube is not sticking and you still have a problem, you may have some serious wear issues.

Lever knobs are available for you lever, try some of the vendors that are into these trucks, (GMC Pauls, Chevy Duty, LMC, ...)
