I think originally the pull cable goes through a hole in the throttle lever with a ferrule with a locking screw on the far side. When you work the throttle the lever just slides over the cable but when you pull the cable the ferrule pulls on the throttle lever.

Some carbs might have a separate lever that the cable pulls that in turn pulls against the throttle lever and pivots on the throttle shaft. I was probably a high school student in auto shop class the last time I worked on one of those around 1965.

I should note that they aren't designed or intended as a "cruise control" they were intended for throttling the engine up to run the PTO when the truck was sitting still.
