I'm not sure on this earlier generation, but the HD drum option for Squarebody trucks utilizes an offset backing plate specific to the wider 2 3/4 brake shoe set-up vs the standard backing plate.
The set-up has the offset backing plate, beefier drum, & wider brake shoes. It's also worth noting that 15" wheels are limited on backspacing when using this combo because the beefier drum can/will cause interference @ the barrel of the wheel.
