The booster, booster brackets, and pedal linkage.
Booster is easy to get through a parts store. The linkage and brackets are harder to find. They must be purchased used. Beware there are two kinds of boosters, Bendix and Delco Moraine. They are functionally the same, but you must make sure your master cylinder is compatible. What I'm trying to say is, Bendix and Delco use different master cylinders. Make sure the booster and cylinder are the same. Best bet is to try and get the old booster from whoever you buy the brackets from so you will know which to get.

There is a bellcrank linkage that goes between the pedal and the booster. It has pivots and such.

There are support brackets that run from booster to fire wall. Most trucks have one some have two, its probably a good idea to get one. You will see how much weight is hanging off the firewall once you get it installed. One thing Tx forgot to mention is a check valve and vacuum line and possibly a vacuum fitting for the manifold if you don't have an extra large port on yours.

I'm at work today, so I don't have my camera here.
But, here's a picture I snagged off of the parts board. Look at the two boosters real close and you'll see the linkage between the brackets. It's not just a rod out of the back of the booster like folks assume. It's actually 2 rods with a linkage between them.
