Car Veers left VIOLENTLY when braking. HELP!

It's odd...
Car: 91 G20 3/4 ton Chevy Van with a TBI 350
Problem: When normally driving, and you let go of the steering wheel, it drifts to the right (passenger side of the road). When driving normally and the brakes are applied, it VIOLENTLY veers to the left. Any ideas?
Van has 146k miles on it, I recently replaced the front brake pads/rotors. I don't remember when we had a front end alignment though...I also think my front tires are BAD! They LOOK nearly bald...
