Mirror ID

I could use some help IDing this mirror. It is on my 68, but I believe it is actually the model for a later year truck. Unfortunately, I caught the drivers' side mirror with a column in a parking garage and broke it, so I need to figure out how to describe it and try to find a replacement.

Also, what kind of backing plate do I need? I haven't taken off the door panel, but I it's pretty clear the screws pulled through the plate.

Lastly, is it possible just to replace just the head itself or is it necessary to replace the arm, too? Thanks!

That's a "Sport"-mirror that would be correct for '71 & '72 (maybe other years, I don't know).

There is a backing plate that is inside the door that should come with a replacement mirror. I believe the head is permanently attached to the base and must be replaced together.
