Rearend swap ?

Can the newer 01-03 c-1500 rearend be swaped into the 73-87. My son had his 01 over the other day lowering the rear of his truck and I thought that the rear setup with its disc brakes would be an nice upgrade. So can it work, or what modifications will need to be made?
Also what would be needed to change the 5 lug to a 6 lug on the front if you were able to switch out the rear so that I would have the same bolt pattern.

There are companies that make disc brake setups for our trucks. They make them for 10 and 12 bolt. This probably would be cheaper and easier than trying to find a rear end that with the right dimensions as well as be a easy bolt in.
