Looking at a 91 r3500 2wd with a granny low 4 speed.
What are my 5 speed swap options?

Sticking with a heavy duty trans if you are going to tow with it an nv4500 is the heavy duty 5 spd.

Your only real option is going to be an nv4500 out of a gmt400. The nv3500 probably wouldn't hold up to a 1 ton truck. Could find a clark 5 spd out of a c60 but good luck finding one, plus the final drive difference probably wouldn't gain you much compared to an nv4500.

As far as I know all the R3500s in those years are Cab and chassis just as an FYI. Which means corporate 14 rear, different frame thickness top to bottom (over 9" tall on C&C and like 6" on a regular truck) and different than standard frame lengths on regular cab models. All crew cab cab and chassis models are 56" cab to axle same as any longbed pickup. Single cab models are 60" and 110" IIRC but neither will fit a standard bed. The registration will be more expensive if it is a C&C and registered at a higher than 10K GVWR. The corporate 14 dually is substantially narrower than a standard chevy dually with a dana rear end. Narrower to the point it fits under a fleetside bed and would not accept a regular dually bed. You may have known all that already but I'm a ford guy by raising I know when I bought my 91 I didn't have a clue it was anything but a regular old gmc 3+3 dually with a flat bed.
As far as swapping the NV4500 is the only way to improve on it in a manual without getting into pricey aftermarket or custom stuff. Unfortunately they always go for premium prices even used because of people just like us wanting to swap one in.

If it is cab and chassis the rear end was swapped or it was born as a single rear wheel truck.
Great looking truck and hard to find with a real bed and single rears. The NV4500s come up on CL all the time but they usually need a rebuild if the price is decent. It's really your only option. Sometimes you find them in wrecking yards, but don't plan on it.

It was born a single rear pickup truck. Has a 10 bolt and 1/2t spindles.
