Howdy... I have a 68 and I just used some washers to make up for the lack of indent on the frame. However, if you do a search under this forum, you will find some good info.

I believe mine came out of a late 70's or early 80's truck.

As for the indent, some guys have just banged the frame with a big hammer, others have cut, some use a torch to create the indent. I saw one that had an adapter for a 66 chevrolet. Not sure where he got that but it looks well made.

Here's a bump for you in case someone has more ideas...

Careful using washers to space it out. It actually throws off the steering column line and can cause gearbox bearing wear, etc. Just a little heads up. If it were me I would heat and hammer the indention in. That seems the best way in my opinion.

True, true... In my case, my 68 has somewhat of an indent, just not enough. So a couple of washers was all I needed.

The bracket that I saw someone use on a 66 will certainly bring it out more at an angle. I believe if it was extreme, I would use some sort of swivel connector on the steering shaft like the hotrodders do.

Otherwise, I agree that creating your own indentation is the route to go.
