8" Diameter Brake Booster
Has been modified to fit 67-72 trucks. Bolts right up to the firewall, connects to the pedal, and stock lines fit the master cylinder. Brand new, never installed.

I got it off of a member here a while back, who got it from another member here, who actually did the modifications to it. It was from a (don't shoot!) Fiero. Just so happened to be the correct size booster, with the correct bore master and correct brake line fittings. I have another unmodified, still new in the box Wagner brand booster/master like this one, if anyone is interested. Same price, plus shipping...

Is the modifications to it just cuting and welding the rod and drilling mounting holes into the bracket?

Yes, looks like he also flipped the booster, but I don't know if this was for alignment or to help hide the check valve...
