It appears to be factory built, but probably not a GM factory. This looks to be a coach built vehicle that was produced new in this configuration. The following link shows a bunch of different manufacturers that offered similar custom configuations:

Yeah, that is a stock coach built truck. There was the exact same one in Klamath Falls Or , same color too, prolly a sister truck but it got totalled. I am not a big fan of these as they don't have the rear door and they really cut the bed down.The one I used to see had a burban front seat in it for rear access.
