How to find info for a new SPID ?

My truck didn't have a Service Parts Identification form attached .
Not even a build sheet.
I would like to recreate one.
How do I get the codes that go with the truck ?
Will GM still have equipment listed for the vin ?
And finally where do get a blank SPID sheet ?

Talk to member chipflyer, he can hook you up with the codes and print you a new spid sheet and it will look awesome, here's the one he did for me.

To answer your questions:

1. www.outintheshop.com You can find nearly every RPO code for our trucks.
2. You cannot determine the original equipment options from the VIN.
3. You can get a blank SPID from Ebay or just about any vendor. However, you will NOT be able to duplicate the original font, and it will be obvious that it is "homemade" This is where Chipflyer comes in...he has replicated the font exactly and the result is a perfect SPID.
