What could have caused this

The seat belt question got me thinking when I took the cab apart I found this big gash. I don't think this truck was ever in a accident since frame is not twisted or tweaked.
Also when replacing heater core I broke off a couple bolts but want to find something close to the original bolts. I find carriage bolts and elevator bolts but nothing with the locking tang. Anyone know where to find these I looked at local farm store and nothing

Try mcmaster carr for the heater core bolts. They may have what your looking for.
