Water pump confusion, which one?

So, I have a 1990 V2500 with the stock TBI 350.
When I look up water pumps, there are 2 options and I don't know what is right.
There is a normal and the a "Flat back". How do I know what pump I need?

Visually looks like the flatback gives clearance for something. I have never run across an engine that didn't take the regular one. I'd honestly say I would have to take the old one and compare it.
