Help ID these headers???? Piston's motor...

I need help ID'ing these headers. I know that they are Hooker Super Comp's. But I need the model # so I can order them.

This is Piston's motor, so maybe he will chime in and let me know. I tried emailing him, but it was sent back to me and said his email address does not exist.
