protect-o-plate help!!!!!!

i've got the original protect-o-plate for my truck that's in the back of the manual. but i can't find anyone or any resource that can decode it for me. if i had a camaro, or impala, or some other chevy, i've have answers, but as for a chevy truck... nothing. does anyone have any answers here?

and while i'm here, i've got a second question. where can i find the available paint chips for my '68 (online pics or books)? autocolorlibrary.com has the original PPG color pages, but it's my understanding that more than one company had paint for the trucks, and PPG only had TWO colors for the 68, fawn and anniversary gold.
