Looks like the perch has some corrosion and lost integrity. Should be an easy fix. If that is the case I'd be checking the rest of the suspension, body mounts, etc.

Re: Any one ever see this happen to a rear end?
I had that happen on my old K5. My perches wern't corroded though. It was more on the lines of low gear on the t-case (w/no front drive line), stage 2 shift kit and 300hp full throttle on pavment. Oh and 38" super swampers. I found my weekest link to say the least on repair. Seems the PO put in different pins that were a lil to long. They accualy were making slight contact with the axle tube. (I found some wear). I noticed it imeadiatly when I hit the brakes and herd a loud screach and felt a bump. looking under the truck I saw my pinion angle was about 25 deg. off. I just pulled the axle, beat the perches back close to normal and welded them with 1/4" plate on the top and sides. Then i built new pins to fit correctly. That lasted real good. All the way up till I blew the ring gear teeth off that poor lil 12 bolt. Seams 38's and 12 bolts don't mix. So I went with a dana 60 after that.

Re: Any one ever see this happen to a rear end?
Looks like it rusted and gave way. The blocks put more on it,too. I've found a spring plate on a K/20 rear (leaf) with an ear broken off. Not rusted, just broke a corner off. I found it when I first got the truck. Maybe a u-bolt got real loose or broke. That's the only thing I can figure.
