6 broken wheel studs on the same wheel???

I have been letting my sister use my truck for the last 4 months or so & the other night she calls me and tells me that she is stuck on the side of the road and the truck will not move. she tells me that when she puts it in gear she hears a clunking sound like the transmission is broken. So I think oh s!!T because I know that the tranny is old and ready for a rebuild. When I get there it is exactly as she said so I have her put it in gear and I look under the truck and the rear driveline is moving so I think rear end or axel busted. I leave her ther to go to Home Depot to pick up a socket set to take the rear drive line out and limp it home in 4WD. As soon as I get to home Depot She Calls me and says that the whell fell off. I get back and sure enough the right rear came off and is now wedged between the ground and the fender. I feel around where the wheel studs should be and nothing ?????
How can this happen? My sister has a new baby so i don't think she was doing anything crazy, Plus I live in the city so it has only seen street driving as far as I know.

BTW 84' silverado 4" lift 35's 12 bolt rear axel
