Crew Cab Owners- I need your assistance.

Trying to work out a driveshaft for my crew cab. I have a few shafts on hand, but nothing is close...

First of all can someone confirm that the layout goes like this (starting from the transmission)...
1- Slip yoke
2- U-joint
3- Intermediate shaft (with carrier bearing at the end)
4- Slip yoke
5- U-joint
6- Rear shaft
7- U-joint (at rear diff)

Also, can you give me the following measurements so I can take them to the yard with me (Automatic tranny btw)-
1- rear shaft length from center of U-joint at the rear to the end of the slip.
2- intermediate shaft length from end of shaft to center of U-joint (unless you have one out of the truck, then you can go to end of tranny yoke). Also, for the intermediate shaft, be sure to mention what tranny you are running.

I have a TH350, a TH400 and a 700r4 available to me, so it'll probably depend on what driveshaft setup I find.
