Who Has/Wants Rear Disk Brakes?

I am gathering information on rear disk brakes. Who of you have them on your trucks? What I have found so far is I need the following parts:

1. Rear disk brake kit (rotors, calipers, caliper brackets, rubber brake hose kit and all mounting hardware.)

2. Disc/disc proportioning valve

3. Rear disk brake hoses

4. Rear disk emergency brake cable

Are these the exact parts I need? Are there any more? Do I need another master cylinder or booster? Who has the best prices. I have a front disk conversion on my truck which works great. After my 700r4 is installed I want to work on my rear brakes. Please let me know.
