Rear Discs: How to connect brake lines?

I have finally put on the rear discs but now I need to connect the brake lines.
Maybe a dumb question but how can I do this?
Perhaps if any who has done this conversion already can tell me how they did it.
Did you used a brake hose with banjo bolt? If so, how did you connect the brake ose to the brake line?
Pictures would say more than a thousand words in this case..
