Door speakers without cutting?

My doors are in good shape and I want to put a 5.25" speaker in the door without cutting. I can build a door panel that has a slight build out for the speaker, but I wanted to see some others before I settle on a design. My '69 has no arm rests, so I want to add a rest, maybe change the handles, and add the speaker within the small door panel.

Did '69 come with an optional carpet kick at the bottom, or is that only newer trucks? Post some pics, thanks.
