Voltage question

1972 chev swb w/ rebuilt 350. My old alternator wasnt putting out 12 volts when I turned on light and fan.I replaced it and it was better but still not putting out enough when fan and lights were on. I brought it back to rebuilder and he changed it to a high amp version. I also replaced the voltage regulator. Now when the rpms get high voltmeter climbs past 16 and still rising. Bad alternator?, bad voltage regulator? something else? I checked and voltage reg appears grounded ok,
