Question Stock Oil Filler Extention ?

Can some one hopefully post some pics of an original, unmolested oil filler extention tube on a 350 V-8 motor. I have the stock filler tube extention and hopefully the correct valve cover for driver's side with disc brakes and vacumn boost assist. Valve cover has one 1 1/4" hole, smooth, round with no twist in lug notches for oil cap. It is used with a rubber grommet for the filler tube and a smaller one for the driver's side PVC. What about the passenger side? I know it needs the one small hole for the tube routed up through the breather. Is there also a larger hole for a push-in rubber based breather cap? My truck does not have original 350 motor but being 4x4 like the idea of not having to stretch so far to put in a quart of oil. Thanks for all replies and in-put.
