01 2500 4x4 problems. Please help!!

This is my brothers truck, it's a 01 2500 with the 8.1 litre v8(the big nasty) the problem is when he is driving down the highway the gauges will flicker and the truck acts like it jumps out of gear. He says it will continue to run but not like it should. sometimes he could turn it off and start it back up an would run a few miles and do the same thing over again. Anybody had this problem if so what was the fix? Thanks in advance.
