Master cylinder to booster Q

It's been a lot of years since I took this apart. Sure wish digital cameras were around back then, but I'm really glad we have this board today.

I want to make sure I get this back together correctly. Here's my questions.

1) Does the seal washer go the way I have it, or the reverse? (The other side is all rubber - no metal)

2) Does the boot go on this way? It scrunches up when put into the booster. Is there special way to install this?

3) Lastly, how is the adjuster on the rod used? Is there a measuremnet to make?
