Bad idle =\ ...advice?

Well, contrast to the topic, I dont mean idle when I start the truck, but Ive been having a problem for quite a while now...

Usually doesnt happen unless I travel more than 8-10 miles, but usually once the truck is warmed and I have been drivin for a bit in speeds > 50mph and I come up to the stop I idle real rough (for lack of a better word) the whole truck vibrates and sometimes it dies on me and have to restart...I can usually avoid it from dyin by keepin the rpm's up or shifting to neutral while im at a light (auto tranny) and its even worse when I have the a/c running on it...any thoughts?
