Some Brake Help

Attachment 607282O.k. Did my brake a alittle while ago, then BANG the peddle goes to the floor and I have problems.
So, I disassemble the entire system and discover problems.
First problems passenger rear-- self adjust came undone and was mangled up.
Second problem--I seem to have installed the rear pads incoorectly.

Passenger side has the smaller pad facing forward, drivers side has larger pad facing forward.
