Inner fender install question

Its been a while since I first tore down my truck and now that I'm putting it all back together some small things have me puzzled. For instance, I'm installing my newly painted aftermarket fender wells. I'm using a stainless bolt kit but I'm curious about the large sqaure hole near the back of the inner fender. Clearly it bolts to the cab just below the firewall but the square hole is too big to support a bolt. The bolt kit didn't come with this bolt either. Was there a special bolt with a backing plate?
