Mounting Plate From Axle to Caliper Wrong Size

It's been a while since I've been on here and since I've worked on my truck, but I'm finally getting back to it and to a much dreaded part, running the brake lines and hooking up the brake system. I was starting to mount the calipers today when I noticed the mounting pieces between the axle and the caliper are completely wrong. I bought these brakes like 2 years ago so I'd have to dig way back into my emails if I still even have any of them from when I bought them. It's a 12" rotor on a 12 bolt rear end, used to be 6-lug but re-drilled the axles to a 5. The caliper is a Baer 1-piston, I forgot the name but I can check tomorrow, didn't see it now when I quickly browsed their site, but remember something GT in there.

This is the gap I'm getting. Can I get different mounting pieces somewhere and what kind? The mounting pieces also have the holes farther apart than they are on the axle.

What is this piece on the top? Looks like two of the holes match up to the mounting pieces but I have no clue...
