Smile Lower Moulding Installation....frustration

Does anyone know the 'trick' to installing the front fastener (back nut) on the lower wide moulding mounted on the door? (Drivers side in this case)
I am replacing this moulding on my truck and it is a challenge to get at the back nut on that.
It is well hidden in behind the lower door hinge and I can not, for the life of me, figure out how they installed this originally....unless, it was installed with the door not yet mounted?
Any info on this guys would be greatly appreciated.
