Need Help identifying air breather/valve cover breather

here's what I got... my truck came with this air cleaner(see below) not sure if its factory or not? the lid doesn't cover the air filter completely(is that correct). also what I'm looking for is the PCV/vent tube from the passenger side valve cover to the air breather. the air breather has a small 1 inch square in it where it looks like something may mount into it from the valve cover?? also there is a metal tube coming out of the passenger side valve cover. any help would be appreciated....
